Veterinary Certificate for dogs, cats and ferrets entering the EU
The Veterinary Certificate below should be used for pet cats, dogs and ferrets without EU Pet Passports entering the EU from eligible countries.
Notes on Completion
Sections I, II, III, IV & V should be completed by a private veterinarian who should then sign the certificate on page 2. The Veterinary Certificate must then be endorsed by an official veterinarian working for the competent authority (usually the Department of Agriculture) in the eligible country.
Sections VI & VII should only be completed by a private veterinarian after tick and tapeworm treatment has been carried out. These sections of the Veterinary Certificate should be blank when the certificate is endorsed by an official veterinarian working for the competent authority.
To avoid confusion dates should be entered in this format - 01 July 2007.
This Veterinary Certificate must be accompanied by the following supporting documentation;
- A current rabies vaccination certificate which bears the microchip number
- The blood test results as issued by an approved laboratory. The blood test results must bear the pet's microchip number.
